Bid Description:
Through this Request for Proposal (RFP), the Agency of Human Services, Department of Vermont Health Access (hereinafter the “State”) is seeking to establish contracts with one or more entities for eyeglasses, lenses, frames, cases and associated parts for repairs and replacements for eligible beneficiaries of the Vermont Medicaid program. The successful eyeglass provider/contractor will provide eyeglasses, lenses, frames, cases and associated parts of high quality and durability with consideration of cost effectiveness and customer service.
